Description
Concord Hospital Medical Group is seeking an experienced APRN or PA-C to join an established outpatient family medicine practice at Pembroke Family Physicians.

This is a four-day workweek role with no weekends, no call, and an average of 16 patients per day, designed for providers who want to deliver high-quality care without burnout. You’ll work in a collaborative, well-supported environment that values thoughtful medicine and strong patient relationships.

Responsibilities
- Provide comprehensive primary and preventive care across the lifespan
- Diagnose and manage acute and chronic conditions
- Prescribe and monitor medications safely and effectively
- Educate patients on preventive health and wellness
- Document care accurately to support continuity and quality
- Collaborate closely with physicians, APPs, and clinical team members

Experience
- Minimum of 3 years of outpatient primary care experience preferred
- Experience in family medicine or internal medicine strongly desired
- Comfortable practicing independently within a collaborative team environment

Education & Licensure
- APRN: Master’s or Doctoral degree from an accredited Nurse Practitioner program
- PA-C: Master’s degree from an ARC-PA–accredited Physician Assistant program
- Unencumbered New Hampshire APRN or PA-C license
- National certification (APRN) or NCCPA certification (PA-C)
- Eligible for or current DEA license
