Pay Range
$43.30 - $64.95
The Care Coordination Supervisor (full-time, days) oversees and effectively manages the delivery of social work/case management functions to patients on assigned team. Participates in Departmental planning and administrative activities. Performs duties and maintains interpersonal relationships in a manner designed to portray a positive image of the Department and the Medical Center. Works with team members, department, and hospital management to promote a harmonious work environment. Insures contacts with patients, public, physicians, and other hospital personnel are conducted in accordance with departmental and hospital policies. Assures that daily work practice reflects the hospital key values of compassion, excellence, innovation and integrity.

Education / Experience:
- Graduate from an accredited school of nursing. BSN required. MSN preferred.
- Minimum of 5 years of relevant clinical experience.
- Previous case management experience preferred.
Licensure, Certification and / or Registration:
- Current license to practice as a nurse in the State of North Carolina.
- Certification in case management highly recommended within 2 years of employment in supervisor position.
Essential Functions:
- Effectively facilitates the collaboration of team members in order to evaluate and deliver needed case management/social work activities to patients/families and healthcare associates.
- Provides team supervision including new staff orientation, completion of annual performance review (APR), management of personnel issues, and case consultation.
- Promotes/enhances professional standards of practice and adherence to departmental and medical center policies and procedures.
- Identifies system issues and facilitates intervention eliminate duplication in services and promote progression of patient care.
- Participates in departmental and medical center initiatives/committees/projects to address identified issues and promote enhanced efficiency in the delivery of patient care throughout the system of care.
- Manages a caseload of patients as the need arises and collaborates with the healthcare team to enhance patient outcomes in a cost efficient manner.
- Ensures age and developmentally appropriate patient care is provided in accordance with Age Specific patient care by regularly updating knowledge of growth and development, the aging and strategies to meet related needs.
